If your child will be a NEW Student to Hooks ISD please see the following Enrollment Packets:

Please bring the following documents when enrolling your child at Hooks ISD.
? A copy of the child’s birth certificate
? A copy of the child’s immunization record
? A copy of the child’s social security card
? A copy of the enrolling parent’s driver’s license
? Proof of residency (i.e. water or electric bill)
Students should also have a copy of their school transcript, withdrawal forms. Also please let the campus
know if your student is 504, Sped or GT.
